Police Records Check Volunteer Letter

Police Record Checks

Volunteers need to obtain a Police Records Check (PRC) before they can be on the ice or in the dressing room as a volunteer. We understand that you might be asking why. Because we work closely with kids, our volunteers fall under a category requiring that we take extra care to ensure the safety of our participants.

The type of PRC we need is a "Level 3 - Vulnerable Sector Check".
  1. Download and complete a signed volunteer letter from the OCMHA. You can download one here.
  2. Go to the Ottawa Police Services website: https://www.ottawapolice.ca/en/contact-us/Police-Record-Checks.aspx
  3. Try to apply online.  This is the easiest way but not everyone is allowed to do this.
  4. If online applications do not work for you, then follow their instructions for in-person application,
  5. When your form is mailed back to you, scan and email it to the Risk and Safety Coordinator at riskandsafety@ocmha.ca
If you have submitted a police check to OCMHA within the past three (3) years, you do not have to submit a new one so long as you have continuously volunteered. We will honour the balance of a police check from another organization, to the maximum of three (3) years, if you volunteered continuously there. If you are returning to volunteering after a break of any duration, or your records check is out of date, you will need to obtain a new police records check and submit it.
